National Health Insurance

Individuals who are required to enter health insurance:

1. Those who are not members of other forms of insurance offered by their company (insurance plan provided by corporate health insurance associations such as Kyokai Kenpo [Japan Health Insurance Association], kyosai kumiai [mutual aid association], kokumin kenko hoken kumiai [national health insurance association], etc.)
2. Residents of Ashiya City (those who are registered as residents)
3. Those who are younger than 75 (those who are 75 or older will be entered into the medical insurance system for the elderly)
4. Those who plan to reside in Japan for more than 3 months

National Health Insurance card

• All members of the National Health Insurance will be provided with an insurance card.
• Please show this at the hospital if you receive an examination.
• It is a criminally punishable offense to use someone else’s insurance card.
• Please return your insurance card to the city hall if you will move to a different city or will enter into a different form of health insurance.

Percentages of medical fees paid by those who are members of National Health Insurance

Age	Younger than elementary school age	From elementary school age to 69	70 years or older; younger than 75	75 years or older
Percentage paid for by the member	20%	30%	20% or 30%	Medical insurance system for the elderly

The following fees will not be covered by health insurance.

国民健康保険に入らなければならない人：次の1.～4.すべてに当てはまる人

国民健康保険証

• 保険に入ると、ひとりに1枚、保険証が渡されます。
• 病院で診察を受けるときに、窓口で見せてください。
• 保険証に書かれている名前の人以外が使用すると、法律で罰せられます。
• 芦屋市から他の市へ引っ越すときや、他の健康保険に入るときは、保険証を市役所に返してください。

医療費のうち、加入者（保険に入っている人）が支払う割合

加入者の年齢	小学校入学前	小学生から69歳まで	70歳以上 75歳未満	75歳以上
加入者が支払う割合	2割	3割	2割 または3割	後期高齢者医療制度

次の費用は、保険の対象になりません。
