Procedures Related to Giving Birth, Death, Marriage, and Divorce

Foreigners living in Japan must provide notification to the city in the event of a birth or death. A notification is also required for marriage or divorce, but the documents required for these procedures differ depending on your nationality. Please speak with the Citizens' Section.

Notifications can be made at any time. During times when the Citizen's Section is shut, such as during the evenings on weekdays, on weekends, or on public holidays, notifications can be submitted to the security office on basement level 1 of the North Wing. A staff member will check the content of your notification. If there are no problems with the submitted content, the day you submitted it will be the day it was received. Wherever possible we recommend going to the Citizens' Section directly in order to have the notification confirmed before submission.

日本に住んでいる外国人は、出生（子どもが生まれたとき）や死亡した人の届出を必ずしてください。婚姻、離婚のときにも届出をしますが、国籍によって手続きに必要な書類が違います。市民課に確認してください。

届出は、いつでも提出できます。市民課の窓口が閉まっている平日の夜間や土曜日、日曜日、祝日は、北館の地下1階にある警備室に提出してください。後で職員が届出の内容を確認し、問題がなければ提出した日が受理日になります。届出の内容を確認してもらうために、できれば提出する前に市民課に来てください。

Necessary Items

1. Birth notification certificate (includes birth certificates issued by a doctor or midwife.
2. Maternal and child health handbook
3. The personal identification documents of the person who will be submitting the notification (My Number (Individual Number) Card, Residence Card, Special Permanent Resident Certificate, etc.)

Precautions

Please submit one of the following to the city hall.

1. Documents that confirm the domicile of origin of the parents
2. Where the parents are living
3. Where the child was born

2. 母子健康手帳
3. 届を出す人の本人確認書類（マイナンバーカード（個人番号カード）、在留カード、特別永住者証明書など）

注意すること

次の3つのうち、いずれかの市役所に提出する。

1. 親の本籍地
2. 親が住んでいるところ
3. 子どもが生まれたところ

3. National Pension Book (for those who are members)
4. Nursing Insurance Card (for those who are members)
5. The personal identification documents of the person who will be submitting the notification ("My Number" (Individual Number) Card, Residence Card, Special Permanent Resident Certificate, etc.

死亡届

2. 国民健康保険証（加入している人のみ）
3. 国民年金手帳（加入している人のみ）
4. 介護保険の保険証（持っている人のみ）
5. 届を出す人の本人確認書類（マイナンバーカード（個人番号カード）、在留カード、特別永住者証明書など）

注意すること

次の3つのうち、いずれかの市役所に提出する。

1. 死亡者の本籍地
2. 死亡したところ
3. 届を出す人が住んでいるところ
