COVID-19 Vaccination Certificate (Vaccine Passport)

Please Note

Vaccination certificates will be issued to persons who are fully vaccinated and plan to travel abroad.

Elegibility

Persons who satisfy all of the following:

• persons who are fully vaccinated and plan to travel abroad
• persons who received the vaccination using a vaccine voucher issued by Ashiya City

Cost

Free of charge

Required documentation

1. Application form
2. Passport (copy) *make sure the passport number and name are visible and readable
3. Vaccination voucher (copy) *or a document that shows your individual number (My Number card) or your address (driver's license) if you have lost your vaccination voucher.
4. COVID-19 Vaccination Record Card (copy)

Application

Apply in person

Please bring all the required documentation to the Ashiya City Health and Welfare Center, Kurekawa-cho 14-9, 3rd Floor, Health Center.

Office hours: 9:00am – 5:30pm from Monday to Friday

*The certificate issuance may take about a week after you submit the application and the certificate will be sent to you by mail.

 

Apply by mail

Send all the required documentation plus:

• Identification including your name and address (copy)
• A stamped return envelope with your name and address
