COVID-19 vaccination certificate / ワクチン接種（せっしゅ）証明書（しょうめいしょ） 

Residents with an Individual Number (“My Number”) Card can be issued a digital version of the vaccination certificate for COVID-19. This is limited to cases where the vaccination was recorded in the Vaccination Record System (VRS). Those who have difficulties getting a digital version issued can apply for a paper version. 

マイナンバーカードを持っている人は、スマートフォンでワクチン接種（せっしゅ）証明書（しょうめいしょ）がもらえます。ワクチン接種（せっしゅ）記録（きろく）システム（VRS）に登録（とうろく）されていない人はデジタルの証明書（しょうめいしょ）がもらえません。紙（かみ）の証明書（しょうめいしょ）がもらえます。

(1)Overview / 説明（せつめい）

Residents can be issued a digital version of the vaccination certificate using the official app from the national government and their Individual Number (“My Number”) Card.

アプリとマイナンバーカードを使（つか）って、デジタル接種（せっしゅ）証明書（しょうめいしょ）がもらえます。

(2) Advantages of the digital version / デジタル証明書（しょうめいしょ）の良（よ）いところ

• Certificates are issued at no cost.
  (Applicants are responsible for any additional costs related to internet usage.)
• Certificates can be issued immediately.
• Received certificates can be displayed at any time by opening the app.
  (After receiving the certificate, it can also be displayed in offline mode.) 
• お金（かね）がかかりません（通信料（つうしんりょう）がかかることがあります）。
• すぐにもらえます。
• もらった接種（せっしゅ）証明書（しょうめいしょ）は、アプリでいつでも見（み）ることができます。（オフラインでも見（み）ることができます）。

3. Personal identification number (PIN) (4digits)
4. (Only if traveling overseas) Passport

2 Paper vaccination certificate (if you cannot apply for the digital version) / 紙（かみ）の接種（せっしゅ）証明書（しょうめいしょ）（デジタルの証明書（しょうめいしょ）がもらえない人（ひと））

Those who need paper vaccination certificate should apply at the counter in Health Center (14-9 Kurekawa-cho). 

紙（かみ）の接種（せっしゅ）証明書（しょうめいしょ）が必要（ひつよう）な人（ひと）は、保健（ほけん）センター（呉川町（くれかわちょう）14-9）に来（き）てください。

3. Vaccination Coupon (marked with your vaccination record)
